If I just joined a new Medicare Part D prescription plan, do I need to change pharmacies in order to pay the lowest prices for my medications?

In some cases.  There are two reasons that you may need to change pharmacies when changing Medicare Part D prescription drug plans: (1) your pharmacy may no longer be included in your new Medicare plan's pharmacy network and (2) your pharmacy may no longer be a preferred network pharmacy and instead, your pharmacy is a non-preferred or standard network pharmacy with higher cost-sharing - meaning you will have drug coverage, but you will pay more for the coverage than you would at preferred network pharmacy.

Coverage at Network and non-Network Pharmacies

Each Medicare Part D plan has an individual network of pharmacies that is very large and probably includes over 50,000 retail pharmacies – plus your Medicare drug plan may include a mail order pharmacy.

However, even with such large pharmacy networks, you should always check with your local pharmacy to be sure that it is included in your newly chosen Medicare prescription drug plan’s pharmacy network.

Coverage at Standard (non-Preferred) Network Pharmacies and Preferred Network Pharmacies

Not only do you need to ensure that your pharmacy is a "network pharmacy", but you also want to check to see if you newly chosen Medicare prescription plan provides different drug pricing or cost-sharing for preferred network pharmacies, as compared to standard (non-preferred) network pharmacies.

As an example of different cost-sharing between network pharmacies, the chart below shows the variation between preferred and standard network pharmacies for the 2024 Ohio AARP Medicare Rx Walgreens from UHC (PDP) plan.

Cost-Sharing Variations between Preferred Pharmacy

As an example of how the different cost-sharing could affect you, the chart below shows how the total prescription costs for three medications (Atorvastatin, Clopidogrel, and Esomeprazole Magnesium) can vary between network pharmacies (such as CVS, Walmart, or Walgreens), depending on your Medicare Part D plan.

Bottom Line #2:  If your current retail pharmacy is considered one of your Medicare plan’s non-preferred or standard network pharmacies, your prescriptions will be covered, but you may pay more for your prescriptions than you would pay at one of your plan’s preferred network pharmacies (you can click here to read more).  But, notice that you may even pay different prices at preferred network pharmacies.

  • Medicare MSA Plans combine a high deductible Medicare Advantage Plan and a trust or custodial savings account (as defined and/or approved by the IRS). The plan deposits money from Medicare into the account. You can use this money to pay for your health care costs, but only Medicare-covered expenses count toward your deductible. The amount deposited is usually less than your deductible amount, so you generally have to pay out-of-pocket before your coverage begins.
  • Medicare MSA Plans do not cover prescription drugs. If you join a Medicare MSA Plan, you can also join any separate (stand-alone) Medicare Part D prescription drug plan
  • There are additional restrictions to join an MSA plan, and enrollment is generally for a full calendar year unless you meet certain exceptions. Those who disenroll during the calendar year will owe a portion of the account deposit back to the plan. Contact the plan provider for additional information.
  • Medicare beneficiaries may enroll through the CMS Medicare Online Enrollment Center located at www.medicare.gov.
  • Medicare beneficiaries can file a complaint with the Centers for Medicare & Medicaid Services by calling 1-800-MEDICARE 24 hours a day/7 days or using the medicare.gov site. Beneficiaries can appoint a representative by submitting CMS Form-1696.
