The toll-free number for Member Services is shown on most of the information you received from your Medicare plan, such as plan enrollment or membership information, Summary of Benefits (SOB), Evidence of Coverage (
EOC), monthly Explanation of Benefits (
EOB) letter, and Annual Notice of Change (
ANOC) letter.

If you still cannot find the telephone number for your Medicare plan, please telephone Medicare at 1-800-633-4227, choose the option to speak with a Medicare representative and then ask the representative to provide the contact information for your Medicare Part D or Medicare Advantage plan.
