The Formulary, according to the Centers for Medicare and Medicaid (CMS) is defined as:
A list of drugs covered by a Medicare prescription drug plan (stand-alone Medicare Part D prescription drug plan (PDP) or Medicare Advantage plan that includes drug coverage (MAPD).
A Medicare plan is allowed to change the plan's formulary throughout the plan year as new medications are introduced.
