Niaspan 500MG ER, Niaspan 750MG ER, and Niaspan1000 ER are all
cost-sharing Tier 3 medications on the 2011 Humana Walmart-Preferred Rx
plan.
These medications would have a cost-sharing of a
20% co-insuirance
(you pay 20% and the plan pays 80% of the negotiated retail drug price)
- if purchased the drugs at a preferred pharmacy such as Walmart, Sam's
Club or RightSource.
If you purchased these drugs at other
Humana network pharmacies (like CVS), your cost-sharing would
increase to 37% co-insurance (you pay 37% and the plan pays 63% of the
negotiated retail drug price).

Remember that this Medicare Part D plan has a
$310 deductible that must be met before you receive the Medicare Part D plans cost sharing.
