May 2013 Summary of the Most Recent Updates for the Medicare Part D Plan Formularies (Drug Lists)

Category: Monthly Formulary Changes
Published: May, 28 2013 05:05:46

The most recent (May 2013) Medicare Part D plan formulary data (or drug lists) updates include the addition of 131 new National Drug Codes (NDC) to the Medicare Part D program.  These represent 77 different drugs, many with multiple strengths or delivery forms.

The NDC uniquely identifies a particular drug, manufacturer, strength, and packaging combination. The NDC is often shown on your prescription bottle as an 11-digit code (see the charts below) or could be formatted as 00000-0000-00 where the first set of numbers identifies the manufacturer, the second set of number identifies the product and strength, and the third set of numbers identifies the packaging.

There are many reasons for NDCs to be added to the Medicare Part D program, a summary is a follows:

Reason for NDC Addition Occurences
new delivery form/manfuacturer 4
new delivery form/strength 2
new delivery form/strength/manfuacturer 1
new drug 25
new drug strength 20
new drug with multiple strengths 45
new drug, strength and delivery form 4
new drug, strength and manufacturer 3
new manufacturer 5
new strength and manufacturer 22

The majority of the 131 new NDCs included new drug strengths and in some cases manufacturers. Two new generic alternatives were introduced:

  • VALSARTAN-HCTZ generic equivalent for Diovan HCT® is on 72 PDP and 246 MAPD formularies
  • RIZATRIPTAN generic equivalent for Maxalt-MLT® is on 51 PDP and 197 MAPD formularies

The following 131 manufacturer / drug / strength / packaging combinations (NDCs) were added to one or more Medicare Part D formularies in the most recent updates. You can click on the NDC or Drug Name below to go to our Q1Medicare.com DrugFinder to see the details of how this drug is handled by all Medicare Part D Plans in Florida -- You can then change the state to your state for details in your service area.

  • Disclaimer for Institutional Special Needs Plan (SNP): This plan is available to anyone with Medicare who meets the Skilled Nursing Facility (SNF) level of care and resides in a nursing home.
  • Disclaimer for Dual Eligible (Medicare/Medicaid) Special Needs Plan (SNP): This plan is available to anyone who has both Medical Assistance from the State and Medicare. Premiums, co-pays, co-insurance, and deductibles may vary based on the level of Extra Help you receive. Please contact the plan for further details.
  • Disclaimer for Chronic Condition Special Needs Plan (SNP): This plan is available to anyone with Medicare who has been diagnosed with the plan specific Chronic Condition.
  • Medicare MSA Plans combine a high deductible Medicare Advantage Plan and a trust or custodial savings account (as defined and/or approved by the IRS). The plan deposits money from Medicare into the account. You can use this money to pay for your health care costs, but only Medicare-covered expenses count toward your deductible. The amount deposited is usually less than your deductible amount, so you generally have to pay out-of-pocket before your coverage begins.
  • Medicare MSA Plans do not cover prescription drugs. If you join a Medicare MSA Plan, you can also join any separate (stand-alone) Medicare Part D prescription drug plan
  • There are additional restrictions to join an MSA plan, and enrollment is generally for a full calendar year unless you meet certain exceptions. Those who disenroll during the calendar year will owe a portion of the account deposit back to the plan. Contact the plan provider for additional information.
  • Medicare beneficiaries may enroll through the CMS Medicare Online Enrollment Center located at www.medicare.gov.
  • Medicare beneficiaries can file a complaint with the Centers for Medicare & Medicaid Services by calling 1-800-MEDICARE 24 hours a day/7 days or using the medicare.gov site. Beneficiaries can appoint a representative by submitting CMS Form-1696.
