You can contact the plan's Member Services department to receive a Member ID card.
If you enrolled in a prescription drug only plan (PDP), you can use our
PDP-Finder to see all Medicare Part D plans in your state. Then you can click on the "Benefits & Contact Info" button below the plan name. You will see the Member Services telephone number (if available) along with the Medicare enrollment telephone number.
When you call Member Services, you can confirm your mailing address and request that a new card is sent.
Also ask the representative for your plan's PCN, BIN, Rx Group Number, and Member ID.
You can get your prescription filled using these four numbers.
If you received a "Welcome" letter from your Medicare Part D plan, the plan's PCN, BIN, Rx Group Number, and Member ID should be printed in the letter.
Likewise, if you enrolled in a Medicare Advantage plan, you can use our
MA-Finder to see all Medicare Advantage plans (MAPDs, MAs, SNPs) in your area (ZIP Code area or county) -- once again click on the "Benefits & Contact Info" button below the plan name to see the plan's Member Services telephone number.
You can then call Member Services and request a Member ID card as mentioned above.
If you cannot find your Medicare plan’s contact information, telephone Medicare at 1-800-633-4227 and ask a Medicare representative to provide your with your plan's contact information.
