If you have not received your $250 rebate check during the 45 days after you reached the Doughnut Hole, you should call your Medicare Part D plan to be sure that your prescription drug cost information has been sent to Medicare accurately.
It is also recommended that you contact Medicare (1-800-633-4227) to ensure that your Doughnut Hole information has been correctly received and, if necessary, telephone the Social Security Administration to ensure that your mailing address is correct (1-800-772-1213 or TTY 1-800-325-0778).
