Will I pay less if I use my Medicare drug plan's preferred pharmacies?

Probably.  Each Medicare Part D prescription drug plan (PDP or MAPD) has a national pharmacy network that includes 50,000 to 65,000+ pharmacies and both "preferred" and "standard" pharmacies are included in a Medicare prescription drug plan’s pharmacy network.

However, preferred network pharmacies usually have lower cost-sharing as compared to standard network pharmacies - meaning, you may pay less for your prescriptions at a preferred pharmacy as compared to a standard pharmacy - and you can see some actual Medicare plan cost-sharing examples below.

As an example of different cost-sharing between network pharmacies, the chart below shows the variation between preferred and standard network pharmacies for the 2021 California SilverScript Choice (PDP) plan.

Here is another example of how coverage costs can vary between preferred and standard pharmacies.  The chart below shows the variation between preferred and standard network pharmacies for the 2021 WellCare Classic plan.

The evolution of using preferred and standard network pharmacies

Since 2012, more Medicare Part D plan have been moving toward adopting using different cost-sharing models for preferred pharmacies as compared to standard network pharmacies.

As an example, in New Jersey Medicare Part D prescription drug plans had only seven (7) Medicare Part D plans in 2012 that included standard and preferred pharmacy pricing.  This number of Medicare plans increased to 15 plans in 2013 - and in 2019, 24 of the 26 New Jersey stand-alone Medicare Part D prescription drug plans use preferred and standard pharmacy pricing.

Nowadays, about 93% of all 2024 stand-alone Medicare prescription drug plans (PDPs) will use different cost-sharing for preferred vs. standard (non-preferred) network pharmacies (as compared to 98% of all PDPs in 2023).  More specifically, 668 stand-alone 2024 Medicare prescription drug plans will offer additional cost-sharing savings if you shop at one of their preferred network pharmacies.

Choosing a preferred network pharmacy when using the Medicare.gov Plan Finder


After choosing your medications, the Medicare.gov Plan Finder allows you to choose pharmacies in your local area as a reference to check prices across the various Medicare Part D plan options.

Tip:  If you selected your Medicare prescription drug plan using the Medicare.gov Plan Finder – and the pharmacy you selected is not a preferred pharmacy you will be shown the higher "standard" network pharmacy prices and not the preferred pharmacy prices - so you will need to look closely at the Plan Finder to determine whether your chosen pharmacies are preferred or standard.

The plan deposits money from Medicare into the account. You can use this money to pay for your health care costs, but only Medicare-covered expenses count toward your deductible. The amount deposited is usually less than your deductible amount, so you generally have to pay out-of-pocket before your coverage begins.
  • Medicare MSA Plans do not cover prescription drugs. If you join a Medicare MSA Plan, you can also join any separate (stand-alone) Medicare Part D prescription drug plan
  • There are additional restrictions to join an MSA plan, and enrollment is generally for a full calendar year unless you meet certain exceptions. Those who disenroll during the calendar year will owe a portion of the account deposit back to the plan. Contact the plan provider for additional information.
  • Medicare beneficiaries may enroll through the CMS Medicare Online Enrollment Center located at www.medicare.gov.
  • Medicare beneficiaries can file a complaint with the Centers for Medicare & Medicaid Services by calling 1-800-MEDICARE 24 hours a day/7 days or using the medicare.gov site. Beneficiaries can appoint a representative by submitting CMS Form-1696.
