When a hospital or healthcare facility asks, "Who is your plan provider?", they want to know the name of the insurance company who providing your Medicare Part D prescription drug plan or Medicare Advantage plan coverage.
You can find the name of your plan provider on your Medicare plan Member ID card (for instance, you may read: Humana, Aetna, WellCare, Cigna, or United Healthcare/AARP).
If you do not have your card or cannot find your card or any correspondence from your Medicare Part D plan, then you can contact Medicare at 1-800-633-4227, select the prescription drug option, and then choose the option to speak with a Medicare representative or say "representative" when you hear the menu options.
Once connected, you can explain your situation to a Medicare representative and ask them to look up the name of your Medicare Part D plan provider based on your
Medicare Beneficiary Identifier (MBI) (found on your Red, White, and Blue Medicare card).
