What do I pay in Catastrophic Coverage if I qualify for Medicare Part D Extra Help or the Low-Income (LIS) program?

Catastrophic Coverage · updated Feb 5, 2026

Nothing.  Since 2024, if you are a Medicare beneficiary and reach the Catastrophic Coverage portion of your Medicare Part D coverage, the Inflation Reduction Act (IRA) eliminates cost sharing for formulary Medicare Part D drugs through the remainder of the year.  This rule applies to all Medicare Part D enrollees and not just people qualified for Medicare Part D Extra Help.

Also, since 2024, the Inflation Reduction Act (IRA) extends benefits for the low-income subsidy program to Medicare beneficiaries earning 150% of the Federal Poverty Level and consequently, the pre-2024 partial-LIS designation (previously people earning 135% to 150% of FPL) was eliminated.
